At your first visit, you will need to fill out our new patient registration form. You can either fill them out in the office, print the forms out and fill them out ahead of time, or fill them out through our online form. If you would like to fill them out in the office, we ask that you please plan to arrive 10 minutes before your appointment time.
Expect your first visit to take longer than a typical appointment; generally the first visit can take anywhere between 30 minutes to an hour. At your first visit, the doctor will take a full medical history as well as a full examination in order to establish a proper diagnosis and treatment plan. You can expect chiropractic treatment at your first appointment as long as your exam does not suggest that you require further imaging or if it is revealed that you require further assessment by another medical practitioner. If anything within the history or exam suggests that further imaging would be beneficial, then the doctor will bring you back to the x-ray suite to perform them. If x-rays are required, then you will be scheduled to return at another time to give the doctor time to go over the images and generate a report.
Things to bring to your appointment:
- Insurance card
- Photo ID
- Past imaging studies (x-rays, MRI's, CT's, etc) that may be relevant
- Medication list
